How To Apply (Intake)

The Placement Specialist will meet with you at the DVR office, your home or another convenient location to gather information about your skills, interests and past experiences. It is not a requirement to meet at the ENABLE office in Livingston. The Director will then review the information provided at intake and make a decision about your acceptance for ENABLE services.

You will need the following items for your intake with ENABLE:

  • Copies of your social security card and birth certificate.
  • Copy of Photo identification card that includes current name and address (DMV is preferred)
  • Current Social Security awards letter stating monthly benefits.
  • A list of 2 to 3 references. Include their address and phone numbers and their relationship to you. Former supervisors and/or co- workers are ideal.
